Facelift (rhytidectomy):
Facelift is an excellent procedure to improve the contour of the neck, jowls and cheek areas. We take great care in preoperative planning to optimize repositioning of the sagging cheek fat, tightening of the facial and neck tissues to achieve a natural youthful appearance. The tissues are repositioned vertically and not stretched laterally in the direction of the ear. We do not attempt to stretch corners of the mouth creating the stereotypic expression identified across the room. Rather, with the vertical repositioning of facial soft tissues, natural rejuvenated youthful contours are created. Care is also taken in the design of incisions to minimize scarring and hair location.
Additional procedures are often performed in conjunction with the facelift procedure including: upper/lower eyelid blepharoplasty, liposuction contouring of the cheeks and neck, as well as procedures to improve laxity of the neck muscles.
Eyelid lift (blepharoplasty):
Eyelid lift improves the excessive skin wrinkling or bags of the upper and lower eyelids. The excess skin is removed and tightened along with contouring of the underlying muscle and fat as needed to achieve a more rested and youthful appearance. Incisions are placed along the eyelash line of the lower eyelid and within the upper lid skin fold and usually are nearly imperceptible.
Nasal aesthetic surgery(rhinoplasty):
Cosmetic rhinoplasty is performed to alter its contour and relative proportions. Since the nasal contour must be evaluated as part of the overall facial aesthetic proportions, a number of procedures are frequently performed to achieve harmony such as reducing or elevating of the nasal bridge, narrowing or repositioning of the nasal root between the eyes, and repositioning of the nasal tip. The surgery may require a small external scar as well as cartilage grafts from the nasal septum. Precision is essential: the surgery is customarily performed with the surgeon using 5X magnifying eye glasses, since asymetry may result if there is 1-2mm side to side difference between the nasal cartilage framework. A chin augmentation or set-back may be recommended to reestabilish aesthetic proportions between the chin and nose in lateral profile.
Neck and Jowl rejuvenation:
Necklift procedures include: suturing and tightening of certain muscles, contouring of fat, and skin tightening to improve the redundant skin, laxity of tissues, and/or excess fat in the neck and beneath the chin. Neck and jowl rejuvenation procedures are frequently performed in conjunction with facelifting procedures. These procedures are highly individualized depending on the particular aesthetic improvement desired.
External ear aesthetic surgery:
Procedures include "pinning back" the ear by reconstructing the antihelical fold or by reducing the height of the concha and reconstructing or reducing the ear lobule.
Breast augmentation:
Enhancement of the breast can be achieved by placing a saline or silicone gel implant beneath the breast tissue but above the pectoralis muscle or by placing the implant beneath the muscle. There are a variety of implant shapes and sizes which can be demonstrated during your consultation. The advantages and disadvantages of the subglandular vs subpectoral implant placement and the various implant types suitable for you will be thoroughly discussed during the initial consultation. Currently the implant manufacturer used by Menlo Park Plastic Surgery Center has a limited lifetime warrantee against defects. Discuss this with the Director of Plastic Surgery, Dr. Bagwat, during your initial consultation.
Breast lift (Mastopexy):
If the breast has sagged such that the nipple is below the inframammary breast crease, a breast lift +/- augmentation is customarily performed. Mastopexy usually requires an incision around the areola, continuing vertically to include the inframammary crease. The breast tissue is repositioned superiorly and the excess skin removed. A breast implant may also be utilized if breast tissue has diminished in size, a common occurrence following pregnancy.
"Tummy tuck"(Abdominoplasty):
Abdominoplasty is performed when there is excess skin on the lower abdomen often seen following pregnancies. A transverse incision is made along the pubic hairline and extended laterally. An incision is also made around the belly button and the abdominal skin is tightened. The abdominal muscles may also require reconstruction at this time.
Tummy tuck surgery requires the patient to restrict abdominal twisting and flexing motion for approximately 3-4 weeks until the incision has healed. An abdominal binder is customarily worn for 4-6 weeks until the tissues have healed sufficiently.
Liposuction and body contouring:
Liposuction is effective in removing localized ares of fatty tissue which remain despite efforts of exercise and diet. Areas of excess fat which persist despite exercise include: "saddlebag"/lateral thigh areas, abdomen and "love-handle" area, submentum(beneath the chin). Liposuction is not a technique for weight loss, but is highly effective in contouring localized fatty deposition.
Following liposuction contouring, a compression garment is worn for approximately 4 weeks or until the desired effect is achieved.
Fillers and injectables:
Botox(r) , Restalyn(r), Juvederm(tm), cosmoplast(tm), and cosmoderm(tm) are some of the agents customarily used to improve wrinkles and supeficial contour deformities. Autologous(your own) fat may also be used to reconstruct soft tissue defects associated with aging. Autologous fat grafting is frequently an ancillary procedure during rejuvenation surgical procedures of the eyes and face.
